Contact Jessica and she can provide you with the necessary codes to check with your insurance company. ICD10 codes must come directly from your physician, that is why it is important to obtain a referral first before checking coverage with your insurance.

Contact Jessica to schedule an initial nutrition appointment.

Jessica will send you a few forms to complete. The paperwork must be completed, at least 24 hours prior to the appointment. Completion of forms should take about 15-20 mins.

Meet with Jessica in person or virtually to develop a nutrition plan for a healthier YOU!

Jessica will create a Superbill (medical invoice) after the appointment that you can submit to your insurance company for reimbursement.

Please contact Jessica to discuss out-of-pocket rates for nutrition appointments. She offers 1:1 consults with follow-ups as needed. Accepted forms of payments: cash, checks, credit cards, including HSA cards.

Payment for nutrition appointments must be made at the time of scheduling.
